Feeling pretty foolish help me out

That's cool. I don't see why it wouldn't be "real". For a fabricator, building something like that wouldn't be too tough. The second rear axle likely is just a slave axle and doesn't receive "power". Just a matter of extending the frame, plumbing in the brakes (or not), and reinforce the frame.

That was the only thread that image produced using a Google search. Several pictures on pintrest and Instagram, but I'm not familiar with either so I couldn't dig anything up from those sites.

It looks legit to me, there was a guy around here years ago that added a tag axle to an old Chevy big block truck. It was a single wheel truck, not a dually. Probably not very capable but it was still pretty neat.
 
There's a second gen truck here in Edmonton sitting on a little lot for sale that has a tandem SRW axles and the OEM box stretched with dual wheel wells. From the distance it looks good but I haven't pulled in to look at it up close. I won't deny that I've thought of doing something like the truck above but with a deck on the truck so I can haul three quads, jet boat, quad trailer, generator, fuel, tools and dog kennel on my current truck. Just wouldn't want that for a daily driver though.
Only because I couldn't handle hauling that much with a derated C&C version, or I'd have to modify the engine which I don't want to be in that game anymore.
 
The rear tandoms can be done as there is 2 mini freightliners that have random axels on it that both drive but at that with the loiad on it I'm not sure if the load is all that real on it
