A twin or compound turbo gives you the best of both worlds a small turbo with quick spoolup and a large turbo for high flow. The exhaust from one turbo feeds the other as well as the compressor discharge from one feeds the inlet of another. People who chase large Dyno numbers or wish to tow with low EGT and make big power often use a compound turbo.

On our trucks it seems a modded stock turbo seems to work well providing a bit more flow while maintaining fuel economy and spool up. Turbo Resource is one such vendor. Turbos need to be rebuilt as the bearings wear. My 03 had the stock turbo for 125,000 mi, then a stock 04.5 turbo now a modded stock turbo that has been rebuilt.

Compound turbos are a bigger hassle to install and may cause passenger battery clearance issues. My 08 cannot use the stock group 65 battery on the passenger side so I had to switch to Yellow tops to get enough clearance.

A bigger than stock single will have lag compared to the stocker. My 06 with the Garrett Stage 2 doesn't light good until about 1800 rpm where the stock was pulling hard at 1300-1400 rpm. The stocker runs out of steam around 2100-2200 while garret pulls hard up to 3000 which is a big help when pulling 4-6% grades where I need to run 2400-2600 rpm.

Everything is a trade. As CToney said a modified stock or a slightly bigger (BD Super B, Garret Stage 2...) seem to work well for a lot of people.
