What I did in my 2nd gen was remove the lever, heat and bend it to where I wanted the handle, and taped the threads to a standard size. I forget what size, but it was real close to the metric size. Then I got a couple connecter nuts for all thread, and a small piece of all thread. One nut I ground down so it would fit up inside the handle. The handle fits the taper on the lever, so with the ground nut it feels more secure, no wobble.
Now, from my normal driving position, the handle is right where I can reach it. Much nicer then having to bend over while driving.
I've been told that truck shops carry shift lever extensions, but we don't have anything like that here.
