My friend has 2001 3500 with 5 speed.If he has engine brake [pacbrake] on and pushes clutch in to come to stop truck dies.If he has brake off coming to stop stays running but hunts really hard for idle.Injection pump was changed,several mechanics have not been able to find the problem.Hoping someone may have the answer.Thanks
 
It's likely set or out of adjustment causing TOO much back pressure. I had a BD exhaust brake and a bake pressure gauge to set it up for max benefit. Check and see if this isn't the problem.

Not enough information about the trucks history to offer a reasonable reply. It could be something simple like inadequate fuel pressure, plugged fuel filter, or corroded battery cables or bad major ground.
 
Something doesn't make sense here. If the Pacbrake has not been modified in any way, the engine will run fine with or without the exhaust brake actuated. An idling engine will labor slightly more when the exhaust brake is actuated because exhaust gases will be forced through a fixed orifice. There is a fixed orifice in the exhaust brake butterfly valve that controls back pressure at higher rpm's which allows exhaust gases to pass through when the valve is in the closed position.

Does the engine have the same idle problems when the transmission is in neutral? I don't think your problem is related to the exhaust brake unless the exhaust brake actuator / butterfly valve is severely damaged, the orifice is blocked, or something very unusual is going on.
