At 40MPH the truck has a bad bounce/vibration, if I keep going it goes away and then comes back as a shutter around 60mph and gets fasted with drive speeds. I had the drive line rebuilt and balanced at Blumenthal's and I still have the problem. I know for a fact that both front axle drive shafts are slightly bent, but I did not think that would cause this much bounce. If I pull my goose neck trailer the truck bounces badly around 35mph and keeps doing it to 45mph, the bounce is very bad at this speed and its hard to make it through. Once through it does ok.

Your front shafts being bent will definitly give a bouncy ride. Although it should probably be present at all speeds just change in frequency as speed increases/decreases. Have you tried removing the front diff shaft to see if it's in the cardigan, or u-joints? Also, check track bar joint play, how about the steering dampner shock, and, and, and the list goes on . . . . Do a search here for "Death Wobble" and get ready for some serious research.
 
I have already addressed the "death wobble". This is something else, like tires out of balance, but I just rotated the tires and it did not change anything. The front drive shaft does not spin while driving so thats not it. New shocks, but it did it with the old ones to. everything in the steering system is new. Ball joints are new. I did some driving to day and it seems to lightly bounce in the rear. but then it bounces in front. I can feel a light movement in the steering wheel so that tells me that the front is causing it. bad bearings might be causing it. I will get the tires balanced again and see what happens from there. I am leaning toward just taking the truck in and getting both axles totally rebuilt. I also have a light tick coming from the truck when I make a turn, and I can really here the tires growl when I turn. Sorry for the long post, Thanks for the help.

I see you're running Toyo tires. I had similiar symptoms with my '99 running BFG AT's. Problems started with BFG's and ended with installation of Toyos. Wish I could help.
 
This sounds similar to problems I had. I found out a common problem is the midshaft bearing getting to sloppy. The rubber wears out and lets the shaft bounce around
