I had two that looked like that. . I think they were intakes... I didnt want to wait on new ones, so I took a die grinder with a fine sand paper style die and cleaned the inside and the rod up... I wonder what in the heck makes that happen and if anyone else has had this happen?? I will be trying to buy some used ones somewhere... .
 
It looks like you may have other issues, but here is some guidance on the orientation of the crossheads:

Upper Valve Train Assembly <?xml:namespace prefix = o ns = "urn:schemas-microsoft-com:eek:ffice:eek:ffice" /><o:p></o:p>
Rocker shafts - installed with TEI-xxx toward the front of the engine.
Crossheads – Dot on top side / long slot toward the exhaust manifold.
Valve lash adjustment, each mechanic has a slightly different feel.
- Tried the M11<?xml:namespace prefix = st1 ns = "urn:schemas-microsoft-com:eek:ffice:smarttags" /><st1:stockticker w:st="on">EGR</st1:stockticker> procedure with 6 in. /lb. torque on adjusting screw. Could not move the feeler gage.
- Lubrizol mechanic has an adjustable torque wrench, he try setting valves with it when he gets back to his lab. ( 1 to 15inlb. Adjustable wrench)
All valve train parts are pre lubed with Cummins Premium Blue oil as they are installed in the engine.

Some say the orientation doesn't matter, but they obviously were marked for a reason. Dunno...
