Its not too bad once you figure out where it is. It takes 10mm wrench to loosen the jam nut, then grab the throttle rod and open the throttle and the adjuster screw can be turned by the other hand. Use a mirror to see what you are doing and then its pretty easy to do once you know where everything is.
If your truck has four wheel abs system, You just can't reach it laying on the fender. If I stand in front of the headlight, its a piece of cake with a sledge hammer holding the throttle down .
