Bought my first CTD in 99. 92 D350 club cab, 3. 54 auto. It now had 238K on it; Mom bought it with 175K. So far have had the rear end completely rebuilt (shop screw up replacing pinion seal, not due to age or miles). Pump and injectors rebuilt last year. Original 518, front end, everything else in engine. Can get 20 mpg on hwy with tailwind and light right foot. I used to get 20 in town, and 22-23 hwy.
Last year I had a 92 W250 I bought with 225K. Drove it back from Chicago, 22. 5 mpg with original fuel system. Replaced clutch, transmission to t-case adaptor, brake master cylinder, and a few odds and ends. Traded it for my 91 plus a little cash (to me) in March of this year. This one has been the most trouble, but is still not bad, all things considered- it was a hot shot for 200K pulling a 35' G-N hauoling tractors all over. Pump had been "turned up a bit" by a previous owner. I've had to replace the head gasket (blew after I advanced timing), turbo (no pyrometer, hot shutdowns), throw out bearing, two U-joints. Got the rear end new bearings and another carrier. R&P still good. It needs injectors, and the head has cracks between every injector hole and intake valve seat. Doesnt sem to affect performance or how it runs. They run them on Case tractors with bigger cracks, they guy said at the machine shop checking it. Manifold has shrunk a bit- bolts on each end broke or were close at HG time. It has a few cracks in the body, and it rattles going over 20 down SEMO gravel (boulder) roads, but still gets 19mpg with one injector popping at 500psi and the others about 500psi low. (Cant wait to get new ones and 24 mpg Oo. ) I wont have another vehicle of any kind, except MAYBE a VW or Mercedes diesel. The Cummins Super B rocks!!
