Sounds like your talking about the transfer case. .

The HD one for the 2500 and which is the one that comes in the 3500 basically has a PTO output.
 
I am not absolutely positive, but I think all 6-speed have two PTO cover plates one on each side of the transmission (drivers and passenger sides).

The 99s have the HD4500 five speed. Mine has a PTO mount on each side of the transmission. The 241HD transfer case also has the PTO output.
 
There is no HD version of the transmissions. All of the 5spds and 6spds in the 3/4 ton and 1-ton Rams all have PTO ports on them.

The Chrysler 1999 truck literature lists a new, upgraded 4500 (they call it 4500HD) for 99 model year, and newer I presume. With "heavier synchros" and other internal components. Maybe Chrysler was putting one over on us, but they put it in all the sales literature and product brochures.



I believe they did this because too many people were crunching the third gear synchro, which is very common in the older 4500s.

I tried to be as descriptive as I could in my post by saying there is no HD transmission option available in the RAM. Yes,the standard version 4500 does exsist,but it was NOT used in the Rams. The LD is a popular trans for jeepers because of the 1st creeper gear and the lighter weight. The NV4500 in the 3/4 and 1 ton trucks had always been the 4500HD version. Of course with all parts on a vehicle,upgrades do happen over the years.
