My brother just lost 5th gear in his '94 2WD NV4500. Any recommendations on a reputable rebuild shop near SE Wisconsin? OR, where to purchase a new/rebuilt trans? I searched the TDR, DTR and Cummins Forum Classifieds with no luck. Thanks guys.

5th gear is known to come out! Usually it does no damage, the 5th gear nut works its way loose (All you need to do is take the tail of the trans off to get to the 5th gear).

I believe Dodge,Standand Transmission, Blumenthal Transmission ... have the gear nut with the set screw but the only real fix is fully splined shaft.

Standard and Blumenthal will have rebuilt trans.

Like already said, Just replace the 5th gear nut with the better nut that is out with the set screws and loc-tite and drive it until it comes off again. There should be nothing wrong with the tran's.

Don't mean to horn in on your topic as I've worried about if it will come get me eventually. Different idea. Who has done a switch out to the newer six speed trans? I've been looking for one used for a possible switch and they are pricey and rare. Any thoughts are appreciated.

If you are really thinking about a 6 speed. Look into the ZF6-650 Way better tran's more of them around used and no more trouble to swap in. Still lots of parts around.

Thanks for the response. I don't think I'm familiar with them. What have they been / are in?

My 98. 5 had fifth gear go out with the previous owner. He said he tightened it back up and tack welded it in 4 spots and I've never had any problems with it in the 10 years I've owned it.

5th gear nut repair has been beat to death in the past. The one fix that no one has reported as failing is the fully splined shaft from Standard Transmission. Every rebuild of theirs comes with one. I replaced a lot of NV4500s before I did the G56 conversion. None of them were replaced because the 5th gear came off.

Also with this rebuild / overhaul make sure to use the correct fluid.

I ended up rebuilding my NV4500 after 20K miles due to using Royal Purple, instead of a properly classed oil (GL-4). I am now using Amsoil with the GL-4 classification and have not had any problems.

Just had the same thing happen to a '96 3500 2x4. This is the second time around at 230k, for a bad 5th, not a loose nut (behind the wheel). The last rebuild consisted of replacing the upgraded main shaft with full splines, and fifth gear. 40k later, 3 teeth missing on fifth. The upgraded nut appears to be intact. What is involved with replacing 5th gear again? All other gears work well. I pull a tandem trailer.

What was the outcome and whom/how did you fix fifth? I have just had the same problem again after 40k miles on my '96 3500 2x4.

An other solution is the one that Fifth Gear repair has. Its a splined cup that goes on the shaft and indexes to the gear. You can see it here at--

Not affliated with this outfit, just passing on what I have seen.

5th gear lost teeth

Are you looking to repair the trans yourself or have a shop do it. Standard Transmission and Gear is a TDR advertiser and the only place I go for transmission work. They can also supply you with parts if a DIY is the plan. http://www.standardtransmission.com/

I hope to repair 5th myself to get the truck 2000 miles in a few weeks. I will pull the trans then and go through it with some help. I have the tail shaft housing off and have access to the mainshaft and 5th gear. I remember a bearing problem last time this happened.
