Im looking for an '03 or so and not sure of the best way to identify the engine being the High Output. Thanks for any info to help.

There is a decal on the drivers side of the valve cover near the front. In the block marked H. P. it should read 305.

If you look in the glove box it should list "48RE" if it is an automatic. If memory serves me, the 2003 got the regular output 5. 9 with the 47RE and the 2003. 5 (post 1 Jan 03) got the H. O. (305HP) with the 48RE. The H. O. prior to January 1, 2003 only got the handshaker option. That is if... memory serves me right... . :)

That's the way I remember it also

For what its worth, my engine sticker says 235 HP at 2700 rpm (not HO), and I have the 48RE transmission. Manufacture date of the Cummins is 6-19-03. How would I find out if my truck is considered a 2003 or 2003. 5 and is this even important?

It's not important on your truck, the only time it matters is when the manufacture makes mid model year changes to comply with new emissions regulations.
Such as 90. 5, 98. 5, 04. 5, . 07. 5.
