And this is why the 06 and up trucks benefit more from a good cam than other year models. Here's and odd question: What is the cam timing of the 6. 7 with the heavy amount of EGR. Is it more traditional b/o no "internal" egr?

And the "internal EGR" of the 04. 5 and up trucks comes from the heavily retarded valve timing that leaves exhaust gases hanging around in the combustion chamber as opposed to "traditional EGR" where exhaust gases are plumbed from the exhaust to the intake (like the 07. 5 and up trucks).

Is there any way the dealer could tell if you installed this cam if you were able to do the install without cutting and welding the crossmember?

No, unless they pulled the cam and looked at the timing. Who's gonna do that? Kinda like injectors/tips. The billet Piers cam has the same Cummins part number as stock. They buy blanks from the supplier and then grind them themselves. The regrinds are, of course, the same parts number.

How will the use of this cam affect performance modules or downloaders that advance the timing? Will it be advanced in addition to what the cam added or is it based on something else?

There would be the mechanical advance from the cam and the electronic advance from the injectors. Correct?

ALWAYS change lifters (tappets) when doing a cam swap. These motors use flat tappet solid lifters and wear a matching pattern in the cam lobe and tappet face. Keep lifters with a given cam or replace with new.

Not externally... however, some like the F1 Helix2, use a bolt-on cam gear that would be obvious with the front timing cover removed.

First, when "timing" is referred to on a diesel, it is the timing of the BOI (beginning of injection) of the fuel.

If the cam was changed, and nothing else, this timing should remain the same. The valve timing would obviously change some, but this has no directe relation to fuel timing.

Mechanical mods that advance timing would be Roktech sensor, tone ring mod, etc.

Also, going with a higher horsepower injector, or increasing rail pressure over OEM will increase timing some.
