I've got the same thing going on. I don't shift into reverse as I pull forward in the morning, so I don't know if it does it in reverse.
What I get ***sometimes*** is the transmission will not engage immediately after I shift into drive. It takes about 3-5 seconds before I can drive. If I push the accelerator, it will free rev. One time I noticed the surge you described as I moved forward. It felt like the truck started in second gear for a second or two and then dropped into first.
I had the dealer look at it during the first oil change (free 3 year oil, filter, and lube for military). They said they updated some software, but it still does the same thing. It happens only when the truck is cold. I've started letting the truck idle for a minute or two while I clear the windows and it doesn't give me any problems.
Really weird feeling though to have the truck in drive and not go...
