It was purely a bad injector. If it drizzles fuel instead of spraying it, you get a hot spot on an aluminum piston. As the soft aluminum deforms it can and did scratch / score the cylinder wall. I was concerned that the truck had over-reved and had caused the problem, this was not the case. The truck did have the latest ECM flash when first checked.

tempforce: I never said anything about extra fuel in the oil, although I'm sure some must have been getting in the crankcase with a bad piston. What I did say about fuel was, I bought a lot of extra fuel for an extra round trip to NC to retrieve the truck.
 
Update: Steve White Motors had the truck for 3 weeks to r&r the motor. I have put 4000 miles on the truck since the new short block, mostly towing. Oil change required came on at 700 miles, I reset it and drove it to about 3500 miles and changed the oil at the dealer. Some 400 miles since the oil change and the oil change required is lit up again along with a CEL and a 2509 code and the ETC lightning bolt (electronic throttle control) light is on. Now what? I'm sick of this thing and I've already resorted to letting it sit for up to a month at a time because I can't stand driving a truck with constant lights and warnings.



I'm not certain what's going on with all the codes and warnings, but several people have noted that similar problems are caused by bad connections, especially grounds, around the batteries. Since you've had an engine R&R, you should check every cable and connector and make sure it's securely connected. I'd remove all four battery connections, clean, re-install and grease them to just to be sure. Watch out for any missing or loose hanging ground straps, they all should be in place on both ends.
 
Usually over revving effects the pump and screws it up. And has nothing to do with the internals of the engine. Usually!! or throw a rod.

Good one for a new engine replacement though.
 
what i was referring to, is your short oil change interval. that you may be getting fuel washing past the rings. on the newer engines the multiple injection events have a tendency to dilute the oil, due to the after injection cycle for emissions.

hope your truck gets back to normal asap.
