It was purely a bad injector. If it drizzles fuel instead of spraying it, you get a hot spot on an aluminum piston. As the soft aluminum deforms it can and did scratch / score the cylinder wall. I was concerned that the truck had over-reved and had caused the problem, this was not the case. The truck did have the latest ECM flash when first checked.
tempforce: I never said anything about extra fuel in the oil, although I'm sure some must have been getting in the crankcase with a bad piston. What I did say about fuel was, I bought a lot of extra fuel for an extra round trip to NC to retrieve the truck.
