There's no way to tell aside from tearing into the system. An engine that spends a lot of time idling or short tripping and doesn't have the chance to warm up will by nature use more EGR and be more likely to be dirty vs an engine that sees higher engine loads or even high way driving.
Cummins doesn't distinguish between either scenario so it's not something to lose sleep over unless you want to be proactive. It's a nasty dirty system and could only benefit from a cleaning.
I have changed my CCV filter twice now in 136,000 miles. The only thing I can suggest is to were some Vinyl gloves that you can throw away with the old filter, this will help to keep your hand clean. The filter sits under the batwing (mine is MIA) and the long plastic cover that you fill the engine oil through (at least on my truck). You will need to un-screw this cover and remove a hose on the side of the cover. The filter sit's under this cover.
To turn off the perform service message (at least on my truck) follow these instruction for the CCV Filter.
here is how to reset the message
turn key to on do not start
press brake peddle 2 times
press gas peddle 2 times slowly within 10 seconds
turn off key
It may be different on the newer trucks, but I do not remember seeing anyone post a different message.
