I had no idea I was going to make the TDR Journal with my problem.
But I want to thank everyone who responded for their suggestions.
My original loss of fifth gear seems to have been related to a larger problem. We had to replace the whole clutch assembly and have the five speed rebuilt.
We bought this used truck two years ago when our 89 collapsed on the road. We do not know how it was treated previously. In the two years, we never, never heard any alerting sounds from the transmission. We never grind gears, nor ride the clutch. Yet, we hit a bump in a bridge and lost all five gears.
Opening the transmission, the two stacked gears at the front of the transmission were totally clean of teeth--as if they had been machined. Again no grinding sounds prior. There were very few shavings in the transmission, did someone clean it up before we bought it? Did double clutching coverup or mask the problem?
My mechanic interrogated us separately, to determine how we had been driving, what we heard or did not hear.

And then there is J & J Transmission in Douglas, Arizona.
The owner came in on Sunday to accept the transmission and start tearing it down, order parts Monday morning and have the transmission back to Buddy Wednesday morning.
We were back on the road that evening.
