I have a 01 that tries to cook itself with any signifigant pull. Have much experience with older trucks and never had a problem. Changed several thermostats, cleaned radiator. Dealer has no clue(big suprise). I bugged the service writer about the fan clutch but he said it is ok. Have also gutted thermostat but did not help.
 
Sounds like a fan problem. Is the fan turning on? Do you hear it on hot days while running the AC and traveling slow? If you don't notice it turning on, it's not on. They make a lot of noise and, in my neck of the woods, really stir up the dust.

You don't show whether you have auto or standard. Autos run hotter as you are also discipating transmission heat. Also, define hot and how much you pulling? When I pull 17K lb 5th wheel in 6-7% grade in summer w/ A/C on, my temp gauge has just a hair of white (218' maybe). Just watch EGTs, coolant temps; go light on go pedal and down shift. Also, if fan is working, should make a roaring sound.

Thanks, I would agree that the fan is my problem. I never hear it but am half deaf. The knucklehead dealer insists it is ok but I will change anyway. Also, can you still get Horton fans?
 
The truck is a six speed. The loads are always big. We run some older 12V trucks and they never have water heat problems. The fan works at least some times but I am going to change it. Do the 24V run hotter that the 12Vs? This truck has always been hotter than my 12V trucks, but this is only the second 24V I have had. Both of my 24V trucks have required much more service than the old ones. (I sold the first 24V because of excessive maintenance issues. We thought it was a "unlucky" truck, maybe all 24Vs are unlucky. ) Like the six speed though.
 
Heat

How many miles on the belt? Look at the crankshaft pulley, if the top of the ridges are shiny, the belt is probably worn out. Another way to check the belt is to put a wrench on the big nut on the fan and turn it back and forth, if the belt slips on the crank pulley, the belt is probably worn out. When under power and the fan clutch kicks in, the AC is on and the alternator is charging, the belt can slip when you need it most. bg
