You can bore the block out . 5mm or 1. 0mm o/s or it can be sleeved back to std. The Cummins p/n for the salvage sleeve is 3904166. A special head gasket will need to used if this is done.
As per Cummins Service topic 07T2-8:
" This Service/Parts Topic introduces new service specific cylinder head gaskets for all ISB CM850 engines. ( used in our trucks since 2003 )
These new service specific cylinder head gaskets, Part Numbers 3966342 and 3966343, must be used when the cylinder block has been bored for oversized pistons or when a repair sleeve has been installed into the cylinder block. The new cylinder head gaskets feature a seal ring that has been moved outward to make sure proper sealing is maintained.
If the cylinder block has not been bored or a cylinder has not been sleeved, the service specific cylinder head gasket can not be used.
The two service specific gaskets are identical in appearance, except in overall compressed thickness (grade). Part Number 3966343 is the thin gasket and Part Number 3966342 is the thick gasket. The grade identification tab is located on the exhaust side of the head gasket between the number 5 and 6 cylinders. See Figure 1. Example A identifies the thin gasket, Part Number 3966343 (holes are spaced close together). Example B identifies the thick gasket, Part Number 3966342 (holes are spaced farther apart).
To determine which service specific gasket must be used, you must first determine the grade of the cylinder head gasket removed from the engine being repaired. On original cylinder head gaskets, the identification tab is located on the portion of the head gasket corresponding to the front of the engine on the intake side. The thickness of the head gasket can be determined by the number of holes in the cylinder head gasket grade identification tab. See Figure 2. There is one hole for thin head gaskets (A). There are two holes for thick head gaskets (B).
Engines originally built with a Grade A gasket (thin) must be assembled with the Grade A service specific gasket, Part Number 3966343. Engines originally built with a Grade B gasket (thick) must be assembled with the Grade B service specific gasket, Part Number 3966342.
CAUTION
Failure to install the proper grade of cylinder head gasket can result in poor engine performance or engine damage.

If the original cylinder head gasket removed from the engine is unavailable (was discarded during engine tear down), then piston protrusion must be measured to determine which grade of gasket must be used. Refer to Procedure 002-021 (Cylinder Head Gasket) in Section 2 in the ISB, ISBe, ISBe4, QSB4. 5, QSB5. 9, and QSB6. 7 (Common Rail Fuel System) Service Manual, Bulletin 4021271. "
How you tell which one you need is to examine the one that came out of the engine and use the same type, thin or thick. It does NOT come in a head gasket set and costs more than the set itself.
