The ramp I made is 38"x16" the bottom edge doesn't sit on the ground it has legs and is about 7"-8" off the ground . That is where the ramp starts . It ends on top of the side steps where the top legs fit between the steps and the truck holding it securely in place. It's carpeted and has cross pieces for additional traction such as the ramps used in dog agility courses .
Ramp only protrudes as far as the door opens

Over many years we've had seven different German Shepherds and eventually had to cope with this issue with all but our most current one. Ramps would be the easiest but it depends upon the dog. Some don't want to have anything to do with it. Both my truck and my wife's Jeep have running boards. They help a lot. The dog puts their front paws on the running boards and we lift their back ends up wheelbarrow style just like p-bar said. They walk their front paws along the floor and onto the seat.
 
Back Seat Labs

6 yr old black lab- i open the passenger side rear door before I let him out of the house, he bolts for the truck and jumps right up onto the back seat. getting out of the back seat, I have to body block &
remind him to step down off the seat onto the floor before he jumps out.

12 yr old yellow lab- she walks up to the passenger rear door and looks at me. I lift her up onto the the rear seat and she stays in the middle. Getting her out is more complicated- if she hasn't turned around I need to open the door she's facing (drivers side) and coax her closer to the edge so I can lift her down. Sometime she turns around and faces the passenger side rear door and I can get her out that side.
Then there are the times she just lays in the middle of the back seat and refuses to move, now I get to climb into the back seat area and move her closer to the edge so I can lift her out of the truck.
When I get her down she trots away to wherever we're going.
 
We have a 7 year old 80# mutt rescue dog that we bought as a puppy. I've always let her jump into the truck, but never out. I pick her up and set her on the ground. She's a pretty amazing dog. One day a few years ago, we were standing around the bed of the truck. I have a 3500 srw 4x4 with 35" tires by the way. My dog walked towards us and jumped over the bed side and into the back of the truck. She's done it probably a hundred times now and I'm still amazed every time I see it.

What's the paint look like?
After a long absence we'll be getting a rescue dog soon.
 
I know this is a bit late but... I have 2 Great Danes one is 12 years old and 150 lbs and a 2 year old 190 lbs after many failed attempts to purchase a set of steps, we finally found one that is heavy duty and not flimsy. It’s the Pet Loader and comes in 2 sizes. I have a 5inch lift and got the taller set probably didn’t need to but it’s worth the price
