I bought a 98 12v in CA and contemplating driving across country. Although it has 260k miles, it still has the CA emissions with the EGR. Does anyone have any experience on how I can expect this to effect the fuel mileage?

It is a shortbed x-cab, auto, 3.5/1 axle ratio, stock tires, 4x4, no lift or big bumpers.

It depends, on a LOT of factors.
How you drive it, wind, terrain, fuel, previous mods, load.
List goes on and on.....
10-20 mpg's in real life.....

I have had four 1st gens that got about 15 to 18mpg. Figured the same the 2nd gen but was concerned with that intrem California EGR system and what effect it had if any.

Sorry, I didn't realise you've owned a couple of these before.
I've never seen one from Ca with the EGR.
I wouldn't expect it to make much of a difference.
MAYBE 10%? Probably less.
I'm think if you get a 49 state manifold and plug the line that should be it for a mod.
Maybe bump the timing a little. That seems to help MPG's on just about all stock engines.
My timing is @ 18 degrees, I have 265 tires and it's a 4x4 with a 5 speed. If I behave I seem to get about 21`ish (corrected for the tires) mpg's in warm weather with good fuel. I have a 60 gallon bed tank and a full tool box. My truck crosses the scales @ 10K lbs.
All filters have just been changed.
The auto is I'd guess -1 mpg penalty.
If you got less than 19 or more than 24 I'd be surprised.

What's easiest and helps mileage the most? 8 oz. of fuel treatment (lubricity, cetane, cleaner, etc., such as Stanadyne Performance) plus 8 oz. of 2-ethylhexyl nitrate per tank of fuel. I hauled a 6x12 Uhaul trailer from VA to WI and back at 70-75 MPH and broke 20 MPG a time or two; that's at the stock 14.5° timing. I'd never gotten over 19.6 i the past, even when I ran 3 oz. of acetone per tank years ago on the trip Dad and I made from here to Vegas, San Fancisco, Houston, then home. (The fuel burned better, but didn't help mileage much. Wait, I was running 80MPH most of the way; maybe it *did* help mileage.)
