They do both, scan the reg and plug in to the obd. any trouble codes emissions related or not will result in a fail. The manual input of false information results in hefty fines and suspension of license... not to many shops willing to do anybody a favor anymore...
 
Where are you? I think only the cars in the NY Metro area (emissions) get it. My wifes jeep throws a code for the auto transmission neutral safety switch and I have to clear it and leave time for the computer to erase it before I get it inspected. The guy knows I fix all my cars myself so he will not make any money by BS'ing me about the check engine light.
 
For a number of years on LI, I was able to avoid emission inspections on my Dodge as it was registered for its empty weight, which was under 8500#. However, in 2008, emission inspections were reconfigured to reflect max GVW regardless of registered weight, and emission inspection became necessary. It is very easy to pass, the problem is finding an inspection station and making the appointment.
 
For years i brought it to local diesel shop and they allways did emissions, never had obd hookup. They changed owners and i went to another place my friend reccomended, they dont do diesels but did mine with the weight change..... i live on L. I... the emissions was called opacity test.
