2nd Gen Non-Engine/Transmission The Clunk

My buddy has a '96 2500 that has a clunk just after take off in 2nd gear. The truck has to roll a few feet before it does this. He has checked all the usual suspects like the u-joints and transfer case mounts. Could it be the fifth gear issue with the NV4500? Maybe the limited slip going out? Thanks.
