2nd Gen Non-Engine/Transmission DSS Install Problem

I am installing my DSS this afternoon, everything proceedeing smoothly until one of the thread inserts on the sway bar mount has either strpped out or dislodged. Either way there are no threads to grab on to. Any suggestions on where to go from here?
 
That might have a nut that has a ear tag to hold when you tighten it up. . It might have slipped away when you took it off
 
The threaded nut that sits inside the frame channel didn't appear to have anything sticking outside the hole. I think it strpped out. Maybe I will need yo weld a nut inside their. I don't see any access onside the frame channel?
 
Maybe put a longer bolt back through in opposite direction.

Since you have no access to the bolt head to keep it from spinning when tightening, you might try this: Before inst. the longer bolt, grind opposing 1/4" flat spots at the end of the new bolt, that way you can hold with Vicegrips untill all is tight.

Also, what type nuts, threads, etc. do the other 3 bolts use? They should all be the same.
 
I think these are threaded inserts inside the frame channel. They are where the stabilizer brackets are bolted to the frame rails. On the DSS you remove the brackets and slip the DSS bracket between them and reinstall using the longer bolts that come with the DSS.
